27" VideoWall monitor by Dotronix, this variant first launched in 1999. (There are multiple variants of this model which use different chassis. To distinguish them, I've dubbed this model as "DSV27 '99") Has RGB over a DB-9 connection, and can handle CSYNC and sync-on-green. Website: Dotronix.com

This particular unit was manufactured after 1999, as it uses a Philips tube that was introduced that year. The first DSV Series monitors were introduced sometime in the early 90s, possibly 1993. For example, this unit from 1993 uses a Philips A68AEC01X tube with a slightly different chassis:
