Summary
Rack-mounted unit containing three 5" Trinitron monitors. Each can accept composite video, with the option of external sync. The predecessor of the PVM-5310.

Documentation for this monitor is not available online at this time.
Notes
Each monitor unit can be removed from the base for service. However, they are not meant to be used independently. The front bezel is attached to the base unit, so you'd have to create a replacement in order to use one on its own.
These monitors use an older style Trinitron tube, with the H-Stat wire integrated into the anode cap. It's not compatible with the PVM-5310 or 5041Q.
