Summary
Medical monitor manufactured by Totoku Electric Co for Toshiba America Medical Systems. Features a very high TVL shadowmask tube from NEC Japan.
The bottom features a removable swivel mount base with a diameter of 1.75". I couldn't find the orignal base, and 1.75" diameter is uncommon for metal pipe, so I opted to remove the base. The natural angle of the case compliments being on a table surface quite nicely.
Notes
Only supports 15kHz RGBHV, despite being full of ICs that support higher scan ranges such as the Mitsubishi M51387P and LA7850. To use regular RGBS sources like game consoles with this monitor, you'll need something that does sync conversion. Most Extron RGB interfaces handle this (I tested with an Extron RGB 109 Plus).
Shares the same RGB jungle as the Nanao MS9-29 multisync 15/24kHz arcade monitor - but this Toshiba does not appear to support anything higher than 15kHz sync.
Sunshade is detachable - simply slide the two black retention tabs on the top towards the edges of the monitor until they are removable.
Power supply is designed by SanKen (SRS-109-1REV). Visually it looks similar to the SanKen power supplies used in NEC monitors like the Multisync 1 and II during this time.
The only controls on the outside are brightness and contrast. Inside the monitor you will find potentiometers for V-Size, V-Position, V-Linearity, H-Size (coil), R/G/B drive/gain, Sub Bright, Sub Contrast, H-"Paise" which appears to be H-Phase - you can use this for H Position, and there is G2/Focus on the flyback.
The stock width and height seem to be quite small. It is possible to push width and height to take up most of the screen (as seen below) but it it seems to cause some linearity issues along the edges so this may not be desirable. Be careful not to turn the width coil too many times, if you drop the slug out from unscrewing it too far it will damage the horizontal output circuit.
